Documents Required For Admission

• Completed Application

• Background Check Authorization

• Application Fee – $75

• Proof of Employment or Prof of SSI OR SSI Benefit’s

• Copy of your Reward letter if applicable

• Last two check stubs / If it applies to you

• Copy of your photo ID

• Copy of Social Security Card

We do not accept sex offenders, indecency with a child or aggravated convictions, family violence may be considered depending on the circumstances.

Application will not be processed without these documents.
